Extremely long half life (40-55 days)
Warfarin - Coumadin DDI
- R-enantiomer major substrate of CYP3A4, 1A2
- S-enantiomer major substrate of CYP2C9
Digoxin - Lanoxin DDI
P-glycoprotein substrate
Diuretics - increase risk of digoxin toxicity (due to potassium depletion)
Phenytoin - Dilantin DDI
Major substrate of CYP2C19 & 2C9
Phenytoin induces CYP1A2, 3A4, 2C9, 2C19
Phenobarbital DDI
CYP2C19 substrate
Phenobarbital is a CYP1A2, 2C9, 3A4 inducer
Lithium
- ACE-Is, ARBs, diuretics, NSAIDs - increase risk of lithium toxicity; lithium use with NSAIDs is contrandicated
Theophylline DDI
- Major substrate of CYP1A2; cigarette and marijuana smoking may reduce theophylline levels (smoking induces 1A2 enzyme)
Ritonavir DDI
major CYP3A4 inhibitor
