What’s the procedure ?
the pp was informed of how she could gain or be fined tokens
the value of each token (poker chip) was such that it could be traded in to purchase better sleeping facilities , watch tv , go on trips etc
pp received token from stad at hospital when she was observed smiling
social reinforcements were also given alongside the token such as ‘well done’
a fine which resulted in the removal of a token also applied if the pp cried .
staff ket records of the tokens in a bank account and the pp was allowed to trade in tokens for desired things or to pay fines
in total the procedure lasted 20 weeks with various other more complesx elements being introduced throughout the week
What are the measurements ?
the smiling response - a slight opening of the mouth and upward turning of the lips
crying response : tears coming from the eyes for 5-30 mins
longer than 30 mins = new cry
responses were recorded by staff 3x a day with each rating being a 2 hour maximum duration - arranged in conjunction with meal times
observers were at least 10 ft away when making recordings
inter rater reliability was used with more than one person recording responses a .95 coefficient was found across ratings
what were the results ?
at baseline - pp was recorded smiling 0 times and crying 30 times
in final week of treatment the pp was recorded smiling 27 times and only crying twice
follow up at 14 months indicated that the patient had n recommendation for hospitalisation or further treatment
