Why is epinephrine sometimes added to a regional anesthetic?
To extend the duration of the block (it causes vasoconstriction at the site of the block)
Why is dextrose sometimes added to regional anesthetic?
It may be added to spinal blocks to extend duration (makes CSF heavier)
Why is sodium bicarbonate sometimes added to regional anesthetic?
It reduces pain on infiltration and extends duration of the block
What does L.A.S.T. stand for?
Local anesthesia systemic toxicity
How does L.A.S.T. occur?
LAST can occur if an excessive dose of local anesthesia is absorbed systemically or if it is accidentally injected into the vascular space
What are CNS symptoms associated with LAST?
- dizziness
- confusion
- tingling/numb lips
- blurred vision
- metallic taste
- seizures
- hallucinations
- loss of consciousness
- respiratory arrest
What are cardiac symptoms associated with LAST?
1.tachycardia
2. bradycardia
3. ventricular dysrhythmias
4. hypertension
5. cardiac arrest
How is LAST treated?
anti-arrhythmics, fluids, oxygen, airway support, lipid emulsion therapy, benzodiazepines for seizures
How does lipid emulsion therapy work?
lipids pull the local anesthesia out of the cardiac tissue
