These are necessary for the continuation of delivery of
family health care services and its evaluation while evaluation of family health services is necessary to identify
the new and continuing family health needs.
RECORDS
These records include information based on factual events, observation
results or measurements taken such as height, weight, body circumference or laboratory examinations carried out like hemoglobin, urine test, stool test and sputum examination depending upon the problem of the family.
FAMILY RECORDS
These records refer to forms on which information about an individual and family is noted.

These are tools of communication

These should represent a comprehensive, systematically organized data and information that are essential for nursing care decisions.

This is found to be time saving, economical and also it is helpful to review the total history of an individual and evaluate the progress of a long period. (e.g.) child’s record should provide space for newborn, infant and preschool data.
CUMULATIVE / CONTINUING RECORDS
The basic unit of service
FAMILY
All records, which relate to members of a family, should be placed in a single family folder. This gives the picture of the total services and helps to give effective, economic service to the family as a whole.
FAMILY RECORDS
The records may be grouped to:
- Age of family member
- Health care requirement cards as per health conditions and morbidity status
Different systems may be adopted depending on the purposes of the records and on the merits of a system.
FILING OF RECORDS
Records could be arranged in the following ways:
- Alphabetically
- Numerically
- Geographically
- W/ index cards
It provides indication of the total volume of service and type of cases seen.
REGISTERS
This summarizes the services of the nurse and/or the agency and may be in
the form of an analysis of some aspect of a service.
These are based on records and registers and so it is relevant for the nurses to maintain the records regarding their daily caseload, service load and
activities.
REPORTS
One of its objectives is to provide a summary of data on health services delivery and selected program accomplished indicators
at the barangay, municipality/city, district, provincial, regional, and national levels.

The fundamental building block or foundation of the FHSIS
TREATMENT RECORD
This is the document, form, or pieces of paper upon which the presenting symptoms or complaints of the patient on consultation and the diagnosis (if available), treatment and date of
treatment is recorded.
TREATMENT RECORD
It constitute the second “building block of the Field Health Services and Information System (FHSIS) and is intended to serve four purposes
TARGET / CLIENT LISTS
4 purposes of of target/client lists
- To Plan and Carry Out Patient Care and Service Delivery.
- To Facilitate the Monitoring and Supervision for Services
- To Report Services Delivered.
- To Provide a Clinic-Level Data Base which can be Accessed for Further Studies
FHSIS Reports constitute the only mechanism through which data are routinely transmitted from one facility to another in the revised FHSIS.
TALLY / REPORTING FORMS
The majority of FHSIS reports are prepared and submitted either _____ or _______
MONTHLY OR QUARTERLY
This is the reporting form that the midwife fills up to report her
accomplishments from the first day to the last day of the month and submits to the nurse at the RHU/MHC for consolidation.
MONTHLY FORM
This form contains the 3-month total of indicators
QUARTERLY FORM
These forms consist of data and indicators needed only on a yearly basis.
ANNUAL FORMS
A report or table will be produced at the PHO (or alternate date processing
site in the province) from the data reported in FHSIS disseminated down to the RHU/MHC and up through the DOH system to the Regional Health Office.
OUTPUT REPORTS
All information related to the client’s history, complaint, diagnosis, services, and/or treatment is contained in three documents or records:
- Individual treatment record
- Target/Client list
- Tally sheet/Report forms

GEOGRAPHIC CODING
These are areas where the population is drawn to receive the healthcare services.
