How can a perviously NPQ person be reinstalled into the program?

A new case will be submitted to the REAB with the following information.
*Current RME after subsequent treatment plan
*Supporting medical documentation with conclusions by treating physician that the individual shows no evidence of residual cancer.
* Dicussion of labs and procedure reports that support afformetioned conlusion
*Treating physician’s plan to ensure the worker remains cancer free
*The submitting facility will address how the patient will be tracked and ensure the treatment plan is being followed.
