When the Block 23 has the physician’s print name, their signature, date, and appropriate Radiation Health Training Credentials (RHI, RAM, UMO).

If an Administrative mistake is made on a NAVMED 6470/13. How does one go about correcting it?

1. One line, Initals, and date the incorrect entry and then write the correction entry next to it.
2. Put comment in block 14 about incorrect entry,

Lastly, if the patient, or provider is still there have them do it again (best but unlikely option).
