Definition
ICD: The inability to control ejaculation sufficiently for both partners to enjoy sexual interaction
DSM: A persistent or recurrent pattern of ejaculation occurring during partnered sexual activity within approximately 1 minute following vaginal penetration and before the person wishes it.

- emotional stress
- history of traumatic sexual experiences
- medication and recreational drugs
- prostatitis
- MS
- thyroid disorders
- varicocele
Physiological causes
- Genetic susceptibility (Neuroreceptor sensitivity)
- Penile hypersensitivity
- Hyperthyroidism
- Prostatitis
- Co-morbid sexual problems e.g. ED
- Sympathomimetic medication
Psychological causes
- Anxiety states
- Early learned experiences
- Lack of experience/infrequent sexual activity
- Psychosocial and environmental factors
- Relationship issues
- Partner issues e.g. pain
Sexual Treatment
More frequent sex/masturbation
Using a condom
Sex with the women on top
Medical treatment
Topical local anaesthetic (e.g. stud 100 spray, lidocaine gel)
Dapoxetine (SSRI)

Stop/start technique & Sensate Focus
Practice ‘point of inevitability’
