What is an uncal/transtentorial herniation? What Sx?

A

Uncus (part of temporal lobe) gets pushed through tentorial membrane against the midbrain.

III nerve palsy - dilated ipsilateral pupil, opthalmoplegia (down and out), contralateral hemiparesis (cerebral penduncle), coma if compression on midbrain

A

Pressure in the posterior fossa forcing cerebellar tonsils through the foramen magnum.
