When can sumatriptan be supplied?
If patient has established migraine (either previous diagnosis or by pharmacist diagnosis) you can sell. Cannot if any interactions eg citalopram-refer
Describe a cluster headache and what you would do in OTC case
- last 30 mins, on and off
- sudden, sharp, localised pain around the eye and side of head
- should refer to specialist because OTC analgesic doesn’t help /immediate medical attention
Describe sx of acute sinusitis
- dull localised unilateral pain
- worse when bending over
- can progress to bilateral when severe
How should acute backpain be treated?
- Oral NSAIDs
- if taking ACEi unsuitable for NSAID (AKI risk) therefore give low dose codeine as paracet alone unhelpful for back pain
When would you give ibuprofen gel OTC?
For musculoskeletal pains
(Not really effective for back pain)
When is Imigran Recovery not licensed?
- if pt not diagnosed with migraine
- if taking combined contraceptive
- if under 18 years
When can you not give Nytol one a night / diphenhydramine?
- under 18 years
- if contraindicated (BPH, asthma)
- if other cause of sleep issues ie antidepressant needs adjusting

Antipsychotics
Which antipsychotic is appropriate for a pt with low BP and high BMI?
Aripiprazole
Which antipsychotic has the greatest risk of weight gain?
Clozapine
What are the risks of clozapine?
- initiated in secondary care
- requires lots of monitoring:
• blood lipids and weight
• WBCs
• fasting blood glucose
(For all antipsychotics- prolactin conc)
What is the effect of smoking on antipsychotics?
It increases the clearance of several eg clozapine
What is the most significant side effect of clozapine?
Constipation
- common SE but can be severe even fatal
- cause of hospitalisation
When to refer a pt on clozapine?
CONSTIPATION
- if medium to severe abdominal pain/ discomfort lasting +hour
- swollen/distended stomach (clozbelly)
- overflow diarrhoea
- sickness/vomiting
- absent bowel sounds
- sepsis symptoms
What are the sx of sepsis?
- shivering
- clammy/discolouration
- high temp
- SOB
- confusion/disorientation
