Quinolones MOA
inhibits DNA synthesis
Broad-spec + bactericidal
Quinolones use
lower RTIs, UTIs (avoid in MRSA infections as innate resistance)
Quinolones e.g.
~ ciprofloxacin
~ levofloxacin
~ moxifloxacin (QT prolongation; Hepatotoxic)
~ nalidixic acid (1st gen; uncomplicated UTIs. Avoid in eGFR <20)
~ norfloxacin
~ ofloxacin
Quinolones SEs
- Seizures ~ lowers seizure thresholds
- Tendon damage
- QT prolongation
- Arthropathy; avoid in pregnancy, children, young adults
DISCONTINUE IF PSYCHIATRIC, NEUROLOGICAL, HYPERSENSITIVTY REACTIONS OCCUR
Quinolones interactions
~ increased risk of QT prolongation; ventricular arrythmias (SSRIs, Quinine, Amiodarone, Macrolides, Antipsychotics)
~ increased risk of seizures; Cipro & theophylline
~ NSAID induces convulsions
Quinolones counselling points
~ Driving (impair driving)
~ antacids and zinc/iron ( leave 2h before or after taking quinolone avoid milk with ciprofloxacin)
~ Protect skin from sunlight
