Define Safety

Patient safety
Safety - freedom from accidental injury
patient safety- freedom from accidental/preventable injuries by medical error
Adverse Event
an unintended injury/complication that resulted in disability, death or prolonged hospitalization due to healthcare management, not underlying disease
Near miss
an act of omission or commission that could have resulted in patient harm, but did not reach the patient
(ex: mixed the wrong med, but noticed before giving to pt, or wrong dose but caught on independent double check)
2 most common mistakes in healthcare
- mistakes with medicine (small errors can have huge complications)
- infections that can be prevented (nosocomial infections)
4 types of action errors:
Unintended:
- slip (omission)
- lapse (memory failure)
- mistake (knowledge based)
Intended:
4. Violation (routine violation, acts of sabotage)
RNAOs Patient Safety Position
- patient safety is priority of nurse and all levels
- RNs must protect and enhance health of patient
- RNs must create an environment that supports pt safety
- Patient safety is not merely a mandate
- RNs view patient safety as morally and ethically imperative to caring for others
