Which nerve mediates pupil constriction/
oculomotor nerve (CN III)
What is responsible for pupil dilatation?
Sympathetic nervous system via the ciliary nerves
What is relative afferent pupillary defect?
aka Marcus gunn pupil
swinging light - affected pupil dilates when swinging light to affected eye when it should constrict - signifies lesion anterior to optic chiasm i.e. optic nerve or retina
What are causes of RAPD>?
Optic nerve - optic neuritis e.g. MS
Retina detachment
describe the afferent pathway of pupillary light reflex
retina → optic nerve → lateral geniculate body → midbrain
Describe efferent pathway of pupillary light reflex
Edinger Westphal nucleus (midbrain) -> oculomotor nerve
What is Horners syndrome?
disruption of the sympathetic nerves supplying the eye
What are the features of Horners syndrome?
- Partial ptosis - upper eyelid drooping
- Miosis - pupillary constriction -> difference in pupil size
- Hemifacial anhidrosis (no sweat)
What are the causes of Horners syndrome?
- Central - stroke, syringomyelia, MS, tumour, encephalitis
- Pre-ganglionic lesions- pan coasts tumour, thyroidectomy, trauma, cervical rib
- Post-ganglionic - carotid artery dissection, carotid aneurysm, cavernous sinus thrombosis, cluster headache
What lesions cause no anhidrosis in horners syndrome?
