What are the 4 essential areas that should be assessed and measured during pulmonary rehab?
- exercise capacity (times walk tests, timed ADLs, shuttle walk test)
- symptoms: dyspnea and fatigue (BORG scale)
- health-related quality of life: self report tools and behavioral surveys
- psychosocial status: anxiety or depression measured by questionnaires
Which patients are candidates for referral?
-Obstructive diseases: COPD, CF, bronchiectasis
-Restrictive diseases: idiopathic pulmonary fibrosis, sarcoidosis
-those at high risk for COPD: smoking, air pollution, lung infections
-chest wall defects: spinal, obesity, neuromuscular weakness
-other: heart failure, peripheral vascular disease, stroke
What are contraindications for pulmonary rehab?
-dementia
-unstable cardiac status
-substance abuse
-cancer
-liver failure
-neurologic or orthopedic condition preventing ambulation
What are the different components of pulmonary rehab?
- General care: eval assessment and preventative care (stop smoking, nutrition, avoiding irritants)
- Pulmonary PT: airway clearance, breathing, cough, posture
- Exercise and functional training: energy conservation, dyspnea relief, strength, endurance, group training
- Education
- Psychosocial management: depression, stress reduction
What does the education component contain?
-disease process
-medications: what, how, why?
-stress management: relaxation techniques
-energy conservation
-benefits of exercise
Which breathing techniques are done?
- Pursed lip breathing: control respiratory rate and improve oxygen saturation
- Diaphragmatic breathing
- Airway clearance techniques: percussion/vibration, postural drainage, effective cough, hydration
What are the benefits of pulmonary rehab?
-reduced hospitalizations
-improve quality of life and psychosocial status
-decrease respiratory s/s
-reduce isolation
-improve knowledge and disease management
-improve ADL performance
-increase survival
