What’s pulmonary rehab
Involves warm up, exercise for an hour and education for an hour, evidence based multidisciplinary comprehensive intervention for patients with chronic respiratory diseases which are symptomatic and often have decreased daily activities integrated into the individualised treatment of the patient, designed to reduce symptoms, optimise functional status, increase participation reduce HC costs
Who is pulmonary rehab advised for
Any COPD patient who has some form of functional disability, evidence based treatment for patients with COPD, combines exercise and education, patients with pulmonary fibrosis
Why do you exercise
Increases exercise tolerance, improves muscle strength, reduces breathlessness, fatigue, benefits on quality of life, other general benefits e.g cardio cholesterol, diabetes
How do you select patients for pulmonary rehab
Majority are COPD, will extend to patients with dysponea from respiratory disease in some areas, disabled by COPD no selection based upon age, impairment disability or smoking status, consider significant co morbidities
What is the exclusion criteria for pulmonary rehab
Unstable angina, unstable diabetes, uncontrolled bp, uncontrolled arrhythmia, MI in 6 weeks leading up to PR, cognitive problems, compliance issues
Structure of a pulmonary rehab programme
Min of twice a week, 6-12 weeks, depending on where you lived rolling or cohort programme, muscle resistance training and aerobic training should be offered within a month post discharge
What outcome measures can be used to assess breathlessness in pulmonary rehab
Borg breathlessness scale rating of perceived exertion scale, 6 min walk test, increments, shuttle walk test, endurance shuttle walk test
