What can cause pulmonary HTN?
A
- Left heart disease- mitral pathology, LVF, LtoR shunt
- Lung parenchyma- chronic hypoxia e.g. COPD, asthma, ILDs, CF
- Hypoventilation- morbid obesity, kyphosis, scoliosis, MND

How does chronic hypoxia cause pulmonary HTN?
A
1- Chronic hypoxia
2- Pulmonary vessels may constrict
3- Buildup of pressure due to reduced bloodflow

A
- Dyspnoea
- Fatigue
- Syncope

A
- Raised JVP
- Left para sternal heave
- Loud P2 +- S3
- Murmurs (PR or TR)
- Ascites and peripheral oedema

A
- Hypoxia
- Hypercapnia
Respiratory acidosis

How is cor pulmonale managed?
A
Treat underlying cause
- LTOT, CCB for reducing pulmonary vascular resistance
- ACEi/Diuretics for heart failure
- Heart/lung transplant
