What is pleural effusion?
Fluid in the pleural space
How does it present?
- Pleuritic chest pain
- Dyspnoea
What examination findings are there on percussion?
- Stony dull percussion
What examination findings are there on auscultation?
Reduced breath sounds
What examination findings are there on observation/palpation?
- Trachea may deviate away
- Reduced chest expansion
What will you see on CXR?
- Meniscus
- Blunting of costophrenic angles
- Fluid in the lung fissures
What special test should you use?
Diagnostic aspiration (USS guided)
- Visual inspection
- Protein level
What protein content signifies exudate in a tap?
35g/L
What cause is identified by an exudate?
Infection
What does each of these colours mean on inspection of the pleural fluid?
- Clear
- Yellow/turbid
- Red
- Clear = normal
- Yellow/turbid = Empyema
- Red = Haemothorax
What can cause a haemothorax?
- Cancer
- Infection
Name a criteria for exudates.
Light’s Criteria for Exudates
Name three parameters/values of Light’s Criteria for Exudates.
- Effusion protein/Plasma protein= 0.5+
- Effusion LDH (lactate dehydrogenase)/Plasma LDH= 0.6+
- Effusion LDH= greater than 2/3 of the upper limit
What could neutrophils in the effusion fluid mean?
- Bacterial infection
- PE
- Parapneumonic effusion e.g. abscess
What could lymphocytes in the effusion fluid mean?
- Rheumatoid
- Viral
