Presents w/ fever, headache, and paroxysmal spasms of chest wall muscles w/pleuritis chest pain. Labs often show leukocytosis or increases CK from mm inflammation.
Pleurodynia (Bornholm disease) often caused by Coxsackie B.
Most common bacterial cause of CAP
Strep pneumo
Lobar Consolidation.
Common opprotunitistic cause of pneumonia in pts w/late stage hIV
Pneumocystitis jiroveci
diffuse, bilateral infiltrates prominently around the hila
Hospital acquired pneumonia can often be caused by
pseudomonas aeruginosa
also causes ventilator acquired pneumonia
A pt w/ hx of prior exposure to TB in childhood presents w/ + PPD but no signs of active TB on cxr. No hx of vaccination w/BCG vaccine How do we approach this?
We treat it as latent TB. Prolonged course of isoniazid or rifampin.
Diagnosis of latent TB
+PPD w/ no evidence of active disease on cxr or sputum gram stain is diagnostic for latent TB
