Puerperal pyrexia - def
A
Any febrile condition in a woman where a temp of >38degC or more is maintained or recurs within 14d of birth

Puerperal pyrexia - causes
A
- Uterine infection (endometritis)
- Perineal wound infection
- Breast causes (mastitis, breast abscess)
- UTI
- Thrombophlebitis

A
- Prolonged ROM
- Prolonged labour
- Instrumentation
- CS
- Trauma - episiotomy, vaginal tears

Endometritis - RF
A
- PROM
- Prolonged labour
- CS
- Intrapartum chorioamnionitis
- Internal fetal monitoring (e.g. use of scalp electrodes)

Endometritis - sx/dx (clinical)
A
- Postpartum fever (in proportion to extent of infection)
- Midline lower abdominal pain
- Tachycardia (parallels extent of infection)
- Uterine tenderness
- Uterine involution
**Dx = clinical

Endometritis - mx (3)
A
- Broad-spectrum antibiotics - clindamycin + gentamicin
- Analgesia - NSAIDs
- Refer to ID if women fail to respond to abx

A
Identify source of infection, causative organism and abx sensitivity
- FBE
- Blood cultures
- MSU
- Swabs from cervix + lochia (vaginal discharge after birth) for bacterial culture
- Wound swabs
