What is frailty
Frailty is not an illness, but a syndrome that combines the effects of natural ageing with the outcomes of multiple long term conditions, a loss of fitness and reserves
Often multiple comorbidities, not a specific diagnosed medical condition
Name 5 syndromes of frailty
Delerium, falls, immobility, continence problems, medicine management challenges
who makes up the mdt for frail patients?
GP/nurses, SLT, PT, OT, family, social care, all centred around the patient
What do rapid response teams do?
Rapid respinse from appropriate member of team to provide same day assessment, aiming to prevent admission and set up immediate short term intervention plan
What is the purpose of occupational therapy?
enable people to fulfill their potential as occupational beings. Promote function, quality of life and realization of potential in people experiencing occupational deprivation, imbalance or alienation
What is the purpose of physiotherapy?
use of movement and exercise, manual therapy, education and advice
what is the purpose of SLT?
anticipates and responds to the needs of individuals who experience speech, language, communication or swallowing difficulties
