What are reasons behind why a tendon may have failed healing response?
microtearing, loss of tightly bundled collagen, increased proteoglycan content, neovascularization, absent/minimal inflammation
What are intrinsic factors related to lower extremity tendinopathy?
age- predisposition rather than cause due to anatomy breakdown
body composition, decreased DF, strength potentially
Why does decreased DF cause patella tendinopathy?
increased amount of loading force on knee and at achilles
What are extrinsic factors of tendinopathy?
corticosteroids, training errors, surface, environment, footwear
What are sources of epidemiology?
volleyball, basketball, running, decreased quad and hamstring flexibility, patellar maltracking
What should a patient not do with patella tendinopathy?
total immobilization
What is considered one of best interventions for tendinosis?
eccentric exercise, remind pts it should be somewhat painful as the tissue needs to be remodeled
What is physiology reasons behind eccentric exercises?
increases type 1 collagen, increases tendon stiffness, decreases neovascularity
How long does it take to remodel tissue?
around 3 months
