Dopamine Hypothesis - VERY BROAD OVERVIEW
Too much dopamine - treat with traditional dopaminergenic blockade
Schizophrenia DSM-5
Two or more of Delusions Hallucinations Disorganized Speech Grossly disorganized or catatonic behavior Negative symptoms
At least ONE positive symptom
At least 6 MONTHS
typical antispychotics - basic mechanism and problems
Dopaminergic blockade - The problem is that you suppress the basal ganglia, leading to a range of problems - Dystonia, Akathesis, and even parkinsonism
Atypical antipsychotics - advantage over typicals
Blockade is more specific - mesolimbic system only. Preservation of dopaminergic tone in other brain systems, including mesocortical and nigrostriatal
Atypical Antipsychotics - problems
Weight gain, high blood sugar, high triglycerades, long QTc - leading to Diabetes
Depot Antipsychotics
Advantage - automatic compliance
Disadvantage - side effects are long-lasting.
Brief Psychotic disorder
Lasts from 1 day to 1 month, otherwise the same as schizophrenia
Schizophreniform disorder
Same as schizophrenia, lasting 1-6 months
Delusional Disorder
Hallucination, disorganized thought/behavior, less responsive to meds. Age >40, women > men
Schizoaffective Disorder
Schizophrenia Symptoms AND a mood component - mania or depression. The mood symptoms are the characterizing factor.
Schizoid disorder
Excessive detachment from social relationships - restricted range of emotion. NOT UNCOMFORTABLE
Schizotypal Disorder
Excessive DISCOMFORT in close relationships, accompanied by perceptual distortions / eccentricities.
