What are UAS?
Undesired aircraft states. A condition or attitude which was not intended by the crew
Review countermeasures:
It is done, deal with it! Evaluating and modifying plans as flight proceeds, remaining alert and assertive in identifying
Systemic based countermeasures:
Stall warnings, systems failure warnings, collision avoidance system, ground pro warning sys, SOPS, checklists, briefing, training
Vigilance decrement:
Decrease after 30 mins by a lot
Latent externals:
Lurking, not directly obvious, user unfriendly work enviro, poor cockpit design, instrument layout, aircraft design characteristics, policies, pilot fatigue, optical illusions, sloping runways etc.
External threats:
Distractions, unexpected requests, weather problems, maintenance issues, heavy traffic, unfamiliar aerodromes, missed approach, pressure to meet schedules, in-flight diversions, system failures
Comms errors:
Unfamiliar foreign accents, non standard phrases, bad radio recep
Random error:
no specific pattern
Anticipated externals:
Weather, heavy traffic, unfamiliar aerodromes
How long can you be effectively vigilant?
Up to 30 mins. Any more is rare
Organisational threat:
Deficiency in the infrastructure, organisation while A/C operating, documentation errors, tour of duty problems
Execution countermeasures:
Used to protect against threats, monitor engine, cross-check info, good system and workload management
Environmental threats:
Environment such as weather, A/C comms problems, ground environments like signage or birds, terrain, operational pressure like late arrivals or unserviceability
Internal threats:
Brought to cockpit in person or pilot and crew. Pilot fatigue, team familiarity, language/culture issue, health fitness, pilot experience, recency/proficiency
Threats originate where?
Environment inside or out of cockpit, weather, workload etc
What is the minimum size for layout of letters and numerals in manuals?
4 millimetres
Unexpected externals:
Distractions from pax, in-flight diversions, missed approaches
Sporadic error:
Most difficult to fix. Normally everything perfect then one out of the blue error
Ground nav state:
Attempting to use the wrong taxiway or runway, taxiing too fast
What is the Herzberg two-factor theory?
Motivating factors (job satisfaction, promotion, part of a team, recognition), non-motivation factors (salaries, company policies, work conditions, job insecurity) these are all job related

Pilot actions or inactions, people doing the wrong thing
