- Have you ever felt like you should CUT down on drinking
- Have people ANNOYED you by criticizing your drinking
- Have you ever felt bad or GUILTY about your drinking
- Have you ever taken a drink first thing in the morning (Eye opener) to steady your nerves or get rid of a hangover
SCOFF
- Do you make yourself SICK because you feel uncomfortably full?
- Do you worry you have lost CONTROL over how much you eat?
- Have your recently lost more than ONE stone (14 pounds) in a 3 month period?
- Do you believe yourself to be FAT when others say you are thin?
- Would you say that FOOD dominates your life?
Screening tool: PCL-5
S/S: flashbacks/severe anxiety/fleeing/combative behaviors
*sx must be present for four weeks following trauma
Post Traumatic Stress Disorder
Screening tool: SCOFF
S/S: fear of weight gain/distorted perception of weight/BMI <17.5/hypothermia/bradycardia/hypotension/hypoactive bowl sounds
Anorexia
Screening tool: SCOFF
Self-induced vomiting/use of laxatives/excessive exercise/**at least once per week for three months**/Body weight normally within or above normal range.
Bulimia Nervosa
Screening tool: CAGE (not considered “valid” screening tool)
Risky Alcohol:
Men = >14xweek/>4xday
Women = >7xweek/>3xday
Alcohol Disorder Dx: at least 2-3 cage questions
S/S: mild withdrawal sx 6-24 hrs after last drink/hallucinations 12-24(resolves in 1-2 days)/seizures 6-48/delirium 72-96 hrs/ Wernicke encephalopathy (acute)/Korsakoff (chronic)
Substance Abuse Disorder (Alcohol)
3 mood states: mania (3 or more sx must be present for a week)/hypomania (similar to mania but less severe)/major depression (5 sx in two weeks)
3 or more sx must be present, lasting at least a week:
DIGFAST (mania): distractibility/indiscretions/grandiosity/flight of ideas/activity increase/sleeplessness/talkativeness
Bipolar Disorder
Screening tool: PHQ-9 template/SIGECAPS (quick screening when PHQ-9 is not available)
SIGECAPS: sleep changes/interest/guilt/energy/concentration/appetite/psychomotor agitation/suicide or death preoccupation (most feared/important complication)
Adjustment disorder: Resolves within 6 months
Post-partum depression: within 12 months post-delivery (must have at leasy 5 sx within 2 week period.
Post-partum blues: develops within 2-3 days of delivery and resolves with 2 weeks
**IF started on SSRI’s, MEDEVAC**
Adjustment and Depressive Disorder
S/S: excessive/persistent worrying that is hard to control, causing distress. Occurring more days than not for at least 6 months.
Screening tool: GAD7
Anxiety Disorder
Multiple panic attacks
S/S: DSM-5; Abrupt surge of intense discomfort that reaches peak within minutes. 4 or more of the following sx occur: palpitations/sweating/trembling/SOB/feelings of choking/chest pain/nausea/dizziness/chills or heat sensations/paresthesia/de-realizations/fear of losing control or going crazy/fear of dying
Panic Disorder
Delusions
Hallucinations
Though disorganization
Agitation and aggression
*Sleep disorder is often accompanied with psychiatric disorders (bipolar/depression). First line tx is “sleep hygiene”. If that is in effective consider antihistamine for acute tx, and trazadone for long term use.
Psychosis
Interpersonal issues (tendency to view others as “all good or all bad”)/one moment a trusted confidant and then suddenly seen as cruel or betraying the next
