1. Six or more of following sx have persisted for at least 6 months (only 5 sx required in adolescents and adults, ages 17 yo and older)

a. Fails to give close attention to details, makes careless mistakes (in school, work, or other activities).
b. Loses focus during tasks or play activities (lectures, conversations, lengthy reading)
c. Seems not to listen when spoken to directly (mind seems elsewhere even in the absence of distraction)
d. Often doesn’t fallow through on instructions, fails to finish tasks (starts tasks but quickly loses focus and is easily sidetracked)
e. Has difficulty organizing tasks and activities (poor time management, doesn’t meet deadlines, messy, disorganized, difficult managing sequential tasks)
f. Avoids, dislikes, or reluctant to engage in tasks requiring sustained mental effort.
g. Often loses things necessary for tasks or activities (school materials, pencils, books, tools, keys, wallet, cell phone)
h. Easily distracted by extraneous stimuli.
i. Often forgetful in daily activities (running errands, returning calls, paying bills, doing chores, keeping appointments)

2. Several inattentive sx present prior to age 12.
3. Several inattentive sx are present in two or more settings (e.g., home, school, or work; with friends or relatives; in other activities)
4. Clear evidence that sx interfere w/, or reduce the quality of social, academic, or occupational functioning.
5. No not occur exclusively during the course of schizophrenia or another psychotic disorder and are not better explained by another mental disorder

DSM-5 criteria for ADHD, hyperactivity, impulsivity type

Answer 3

1. Six or more of follow sx have persisted for at least 6 months (only 5 sx are required for adolescents and adults, ages 17 yo or older)

a. Often fidgets, taps hands or feet, or squirms in seat.
b. Often leaves seat in situations when staying in seat is expected.
c. Run about or climbs in situations where it is inappropriate (adolescents or adults: may be limited to feeling restless.)
d. Unable to play or engage in leisure activities quietly.
e. Often “on the go,” acting as if “driven by a motor” (unable or uncomfortable being still for extended time: restaurants, meetings; experienced by others as being restless or difficult to keep up with).
f. Often talks excessively.
g. Blurts out answers before question has been completed (completes people’s sentences; can’t wait for turn in conversations).
h. Has difficulty waiting his or her turn (waiting in line)
i. Often interrupts or intrudes on others (butts into conversations, games, activities; uses other people’s things without permission; adolescents and adults: intrude into or take over what others are doing).

2. Several hyperactive-impulsive sx were present prior to age 12 years.
3. Several hyperactive-impulsive sx are present in two or more settings.
4. Clear evidence that sx interfere with, or reduce quality of social, academic, or occupational functioning.
5. Sx do not occur exclusively during schizophrenia or other psychotic disorder and are not better explained by another mental disorder.
