Spread may be local (seminal vesicles, bladder + rectum), via bone or lymph.

On DRE:
-Asymmetrical hard nodular prostate
Loss of midline sulcus

Investigations:
A
First line - PSA
Then DRE
GOLD STANDARD - transurethral ultrasound guided biopsy
Isotope bone scan to check for metastases
