acute prostatitis
this is inflammation of the prostate usually due to bacteria
what is the most common cause of acute prostatitis in young adults
gonorrhea and chlamydia
what is the most common cause of acute prostatitis in older adults
e coli and pseudomonas
how does acute prostatitis present
dysuria, fever and chills. tender and boggy prostate on DRE. WBCs and bacterial cultures.
chronic prostatitis
chronic inflammation of the prostate. dysuria, pelvic, low back pain. WBCs but cultures are negative.
BPH?
hyperplasia of the stroma and glands. age-related change with no increase in cancer risk. related to DHT.
what zone of the prostate does BPH affect?
the central periurethral zone
clinical presentation of BPH
starting and stopping the urine stream issues, impaired emptying, increased risk of infection and hydronephrosis, dribbling. microscopic hematuria, elevated PSA.
what else does BPH increase the risk for?
hyperplasia of the bladder wall smooth muscle, also bladder diverticula.
prostate adenocarcinoma
malignant proliferation of the glands. most common cancer in men, 2nd most common cause of death.
risks for prostate cancer?
race (african > caucasian > asian)
diet high in saturated fats.
why is prostate cancer usually clinically silent?
because it is in the posterior, peripheral region and thus doesn’t cause symptoms.
