What is a major side effect of cisapride?
Cardiac arrhythmias

What is the MoA of domperidone?
D2 receptor antagonist –> inhibition of the inhibition of ACh-induced contraction
Does domperidone effect brain dopamine receptors?
No…does not cross BBB
What is the MoA of metocopramide?
D2 receptor antagonist –> inhibition of the inhibition of ACh-induced contraction
Does metocompramide effect brain dopamine receptors?
Yes…prevents vomiting…possibly prevents hiccups
What are the side effects of domperidone?
Somnolence, nervousness, agitation, and anxiety
Dystonia, Parkinsonism, and tardive dyskinesia
Increased pituitary prolactin release (impotence, galactorrhea, menstrual disorders)
What is the MoA of macrolides treating constipation?
Macrolides stimulate motilin receptors on GI smooth muscle –> initiation of MMC
Are macrolides often used to treat constipation?
Not really…resistance to the drug develops
