Prognosis Flashcards
Effect of age on recovery
There is no effect of age on recovery. (Wertz & Dronkers, 1988)
Age is not an important factor
does NOT seem to make a difference with stroke induced aphasia
Effect of gender on recovery
Gender does not play a role in incidence of aphasia or recovery
What are some factors that affect recovery?
- etiology
- type of lesion
- site of lesion
- size of lesion
- number of lesions (multi-infract=less recovery)
- type of aphasia
- time post onset
Evolution of aphasic syndromes
Kertesz, 1984
Global–>Broca’s
Broca’s–>Anomic
Wernickes–>Conduction
Processes involved in recovery
Structural preservation Physiological restoration Axonal regeneration Unmasking of latent processing capabilities Substitution of function Cortical reorganization
Aim of Treatment
“help the pt recalim as much as possible of the underlying damaged capacity to process langauge in the same way as a “normal” person processes language” Basso, 2008
Meta-Analysis of Clinical Outcomes in Treatment of Aphasia
Robie - 1998
1) AMount of treatment
2) Type of treatment
3) Time post-onset
Outcome measures = score on comp battery such as PICA
Results of Meta-Analysis
tx in acute period (<3mo) but for moderately impaired still has a large impact in chronic stage
Important to have at least 2hr/week*******
How often??
Meta-analysis (Robey, 1998) confirmed that amount of time in tx was an important factor in determining the success of the therapy
