Engine Malfunction after V1 PM Roles

A

PM will announce “Engine Malfunction”

If reduced thrust t/o has been used PM will call G/A thrust available

Call postive climb

Act on gear call “gear up”

No action until 400ft RA

In response to confirm the failure - State the failure indications

Crew agree failure and course of action

Confirm recall items

Complete remaining recall items

“QRH checklist complete”

“After t/o checklist complete”

PF Engine Malfunction After V1

A

After engine malfunction - raise nose to 12.5* (2-2.5*) per second

Call gear up

Fly at V2 - V2+20 follow FD

EO SID - If speed below V2 +15 then call “bank angle 15 HDG….”

“Confirm the failure”

Crew agree failure and confirm course of action

At the direction of the PF action recall items - if they are required - PF initiates them by disconnecting the auto throttle and closing the thrust lever. This is monitored by the PM and then the remaining recall items are done by PM

“Clean speed”

After Flap retraction - LVL CHG and MCT set - “level change, set MCT A/P A/B to CMD”

  • Close thrust levers
  • Disconnects a/t
  • Speed brake raise
  • Max reverse thrust
  • Brakes apply, monitor RTO
  • Park brake set

  • Confirm failure, call for recall
  • Monitors F/o and manages communication

Decides on next course of action

Decide to evacuate

  • “evacuation checklist”
  • Monitors checklist and communicates with ATC and makes PA
  • “Evacuate. Unfasten your seatbelts and get out!”

Rapid De-pressurisation and Emergency Descent PF

A

Both pilots

  • Don O2 masks
  • Select mic to MASK

Ensure the a/c is flying safely

  • Keep a/t and a/p engaged
  • Keep speed just MMO/VMO
  • Use speed brakes
  • Lower speed brakes with 1000ft to go

Emergency descent

  • Announce “Emergency descent checklist”
  • MCP Set HDG
  • MCP - Set lower alt
  • MCP set LVL CHG and MMO/VMO
  • Thrust levers CLOSE
  • Speed brakes RAISE

Set HDG required
Set 10,000 or MSA if higher

At level off alt

  • Confirm speedbrakes are down
  • T/o O2 mask off if below 10,000ft cabin altitude
  • “Descent is now complete would the senior cc member come to the f/d”
  • Decide on a course of action
  • Obtain ATC Clearance

Check QNH and action recall items
