What happens during the assessment element?
the RN collects pertinent information relative to the patient’s health and/or situation
What happens during the diagnosis element?
the RN analyzes the assessment data to determine the actual or potential problems and issues
What happens during the outcomes identification element?
the RN identifies expected outcomes for a plan individualized to the patient or situation
What happens during the planning outcome?
the RN develops a plan that prescribes strategies to attain expected, measurable outcomes
What happens during the implementation element?
the RN implements the identified plan
What happens during the evaluation element?
the RN evaluates progress toward attainment of outcomes (seeing how it works)

Accountability
professionally and legally answerable for the type and quality of nursing care provided
Caregiver
assists patients. regain health through the healing process, attain maximum level of function, and meet all needs
Advocate
protects the patient’s human and legal rights
assists the patient in asserting those rights when necessary
Educator
formal or informal
utilizes methods that are the patient’s preferences and incorporates the family
Communicator
Central to the nurse-patient relationship
allows nurse to identify the needs of their patients
Manager
provides for collaborative patient-centered care
coordination of unit or facility staff, policy, and budgetary responsibilities
