What has happened in this situation (and why) and what should have happened?
A
Miscommunication between consultant running clinic and GP has meant a failure in continuation of care.
Consultant could have explained course of action to patient so they could chase it up themselves.
GP has not checked letters from consultant
Consultant has not sent electronic copy to GP etc
