A patient is having a fever in response to anesthetic during surgery, what is this known as and what is the cause?
Inhalation of general anesthetics (Halothane) can cause what is known as Malignant Hyperthermia.
Is also seen when a combination of muscle relaxants are used.
Occurs in 1/12,000 pediatric anesthetics.
requesteration of calcium back into the sarcoplasmic reticulum does not occur.
What is the treatment of choice for malignant hyperthermia?
What are the signs?
Dantrolene!! 1mg/kg rapid push.
Hyperventalate, correct the acidosis, cool the patient.
Fever with increased body tnmperature, metabolic and respiratory acidosis, and Hyperkalemia.
What is considered a normal temperature in F?
97.7-99.5
What is considered a normal temeprature in C?
36.5-37.4
What body temp post-op is considered significant?
Body temp >100.4
What is postoperative Heat pyrexia?
Occurs during surgery and up to 1 hour post-op
Caused by trauma of the procedure causing inflammation.
What are the 5 causes of post op fever?
What time period would you see each?
Wind Atelectasis 1-2 Water UTI 2-3 Wound Infection 3-7 Walking DVT 5-7 Wonder Drugs >7 days
What are the proper progression of image orders for pulmonary embolism?
D-dimer > Venogram > V/Q scan + CXR
