How is the Lauge Hansen named?
First part of the name is the position the foot is in during injury.
The second part of the name is the movement of the talus during the injury.
Describe the stages of Suppination-Adduction injuries.
Stage 1: Lateral collateral ligament injury with a transverse fracture of the fibula below the level of the ankle.
Stage 2: Vertical fracture of the tibia.
Note that there is no apparent damage to the deltoid here.
What is the only ankle fracture pattern that ends up with a medially displaced talus?
Supination-Adduction injuries.
If you see an ankle injury in which there is only a vertical fracture of the tibia, what classification must it be?
It must be a stage 2 supination adduction.
What is a Wagstaff fracture?
A fracture noted at the distal aspect of the fibula secondary to a rupture of the AITFL.
What is a Tillaux fracture?
A fracture at the distal anterior-lateral aspect of the tibia due to an ATIFL Injury
What is a Tillaux fracture classified as in the Salter-Harris Classification?
Salter-Harris III
In which the fracture line crosses physis and epiphysis but does not interupt the metaphysis.
What is a Volksman fracture?
this is a fracture of the posterior tubercle of the tibia.
Seen with a PITFL rupture.
Describe the stages of a Pronation Abduction injury
Stage 1: Longitudinal fracture of the medial malleolus with potential deltoid rupture.
Stage 2: Rupture of the AITFL/PITFL (Volkmans, Wagstaff, Tillaux fx’s)
Stage 3:Short oblique fracture of the Fibula at the level of the ankle joint. (Weber B)
Describe the stages of supination external rotation
Stage 1: Rupture of the AITFL (Tillaux Fx)
Stage 2: Spiral fracture at the ankle joint (posterior spiking) (Weber B)
Stage 3: Rupture of the PITFL (Volkmans)
Stage 4: Transverse fracture of the medial malleolus with possible deltoid rupture.
Describe how you differentiate a stage 3 pronation abuction with a stage 2 supination external rotation
Both are fibular fractures at the level of the ankle
When fixating syndesmosis it is important to remain at what height above the ankle?
