Cuff leak test prior to extubation

A

Assess the patient to ensure adequate breathing without mechanical ventilatory support
Suction the patient’s mouth and Airway
Deflate the endotracheal tube cuff
Briefly occlude the endotracheal tube and assess ventilation
Suspect laryngeal edema if the patient is unable to breathe around the occluded endotracheal tube with the cuff deflated

A

Ensure that the patient does not require continued mechanical ventilation support
Ensure the adequate spontaneous ventilation is present
Ventilator weaning indices are positive
Upper Airway obstruction or Airway compromise is unlikely

A

Provide adequate humidification
Insure secretion clearance by providing bronchial hygiene such as suctioning turning chest Physiotherapy
Provide bronchodilator to treat or prevent present bronchospasm
Avoid artificially hyperventilating the patient and creating a respiratory alkalosis
Reduce imposed work of breathing
Rest ventilatory muscles to avoid respiratory muscle fatigue or atrophy

Assess acid-base balance and electrolytes
Treat metabolic acidosis ( kidney failure, lactic acidosis, ketoacidosis)
