What are the DD for a pelvic mass? What is the major thing to be differentiated on an US?

What is the common description in a pelvic mass?

A

Must differentiate ovarian vs extra-ovarian.

Ovarian:

  • cyst (follicular, corpus luteum, theca lutein)
  • endometrioma
  • benign/malignant neoplasm
  • mets: carcinoma.

Extra ovarian

A. Gynaecological

Fallopian tubes: ectopic pregnancy, fallopian tube cancer, hydosalpinx/tubo-ovarian abscess.

Uterus: Pregnancy, Endometrial cancer, Leiomyoma (pedunculated),

B. GI

  • Appendix: abscess/tumour
  • Large bowel: IBD, malignancy, diverticular abscess.

C. Kidneys: pelvic kidney

D. Bladder

E. Peritoneum: cyst/mets malignancy.

Descriptor: “pressure”.
