Management of an abscess
Incise/drain via XLa, tooth or ST’s
First line antibiotic for bacterial infections
Phenoxymethylpenicillin tablets
250mg 2 tablets 4x daily for 5 days
Send: 40 tablets
Second line antibiotic for bacterial infections
500mg Amoxicillin 1 tablet 3x daily for 5 days
Send: 15 tablets
Bacterial infection (abscess) prescription for allergy to penicillin
400mg Metronidazole 1 tablet 3x daily for 5 days
Send: 15 tablets
Contraindications for Metronidazole
Warfarin pts + statins
Avoid alcohol
Second line for Abscesses
Clindamycin
Abx for NUG
400mg Metronidazole 1 tablet 3x daily for 3 days
Send: 9 tablets
Second line abx for NUG
Amoxicillin 500mg 1 capsule 3x daily for 3 days

Rinse after corticosteroid inhaler

Contraindication of Fluconazole
Warfarin + statins
What effect does Fluconazole have on Warfarin?
Increases anti-coagulant effect of warfarin
Inc risk of bleeding
What effect does Fluconazole have on Statins?
Rhabdomyolysis (breakdown of skeletal muscle tissue)
Hepato-toxicity
Prescription for Denture Stomatitis
Fluconazole
50mg capsule 1 capsule daily for 7 days (max 14 days)
Miconazole Oromucosal gel 20mg/g 80 tube
Pea sized amount applied after food 4x daily - use for 7 days after lesions have healed
2nd line prescription for Denture Stomatitis (pts who are on warfarin/statins)
Nystatin oral suspension 100,000 units/ml, 30ml 1ml after food 4x daily for 7 days (48hrs after lesions have healed)
Rinse around mouth hold near lesion for 5 mins then swallow
Causes of angular cheilitis
- In denture pts = fungal infection + assoc denture stomatitis
- Non denture pt = more likely bacterial
Prescription for angular cheiliits
Miconazole cream 2% 20g tube
Apply to angles of mouth 2x daily for 10 days after lesions have healed
Contraindication for Miconazole
Warfarin/statins
Sodium Fusidate ointment indications or angular cheilitis
Only if bacterial cause
Sodium fusidate prescription
2% 15g tube angle of mouth 4x daily
HSV/Primary herpetic gingivostomatitis prescription
0.2% CHX MW - send 300ml 10ml rinse for 1 min 2x daily
If pt in severe pain
Aciclovir 200mg 1 tablets 5x daily for 5 days
Herpes labialis prescription
5% Aciclovir cream send 2g apply to lesion every 4hrs 5x daily for 5 days
Varicella zoster (shingles) prescription
Aciclovir 800mg 1 tablet 5x daily for 7 days
F toothpaste prescription
Sodium F toothpaste 0.619% (2800ppmF)
Send: 75ml
Brush teeth for 1 min after meals using 1cm spit out 2x daily
Sodium F toothpaste 1.1% (5000ppmF)
Send: 51g
Brush teeth for 3 mins after meals using 2cm spit out 3x daily
Ulceration 1st line prescription
CHX 0.2% 300ml 2x daily for 1 min
Benzydamine Hydrochloride spray 0.15% antiitflammatory + analgesic 30ml 4 sprays onto area every 1.5hrs
Ulceration 2nd line prescription
Betamethasone MW (500microg dissolved in water 10ml 4x daily
Beclomethasone inhaler (200 dose inhaler, 1-2 puffs onto ulcer)
Lichen planus topical OTC prescription
CHX MW
Benzydamine MW 0.15%
Avoid SLS
Lichen planus persisting symptoms primary care
Betamethasone MW 500microg dissolve din water 10ml 4x daily
Beclomethasone inhaler 200 dose inhaler 1-2puffs
Hospital setting for Lichen planus
Skin steroid cream - CLOBETASOL (gingival veneer)
Topical tacrolimus
Hydroxylchloroquine
Systemic immunovmodulators Azathioprine
