Preprosthetic Surgery Flashcards
In the absence of teeth, alveolar bone absorbs until there is just ___ bone left, leaving the ___ on top
Basal
Mental foramen
What is an alveolplasty?
If multiple teeth are extracted for dentures, trip the alveolar bone to smooth it. This will allow better fit of the denture and less painful.
If doing implants, keep the bone and bill it with graft.
What is the most common exostosis?
Mandibular tori
How do you remove exostosis?
Open flap
Use rongieurs or pineapple bur to smooth it down
Suture it up
Why are palatal tori difficult to remove?
The overlying mucosa is super thin. Extreme care must be taken to protect the flaps. Bone can be exposed s after suturing because of thin flaps. (Not a big issue)
Potential problems: perforation into nasal cavity. Be sure you’re not going too deep.
Why is it that just removing the epulis fissuratum doesn’t work?
The vestibule reduces
What is a good technique to remove epuli fissuratum?
Do a SUPRAperiosteal disection. Then suture the flap way up high to the periosteum, leaving the vestibule at proper depth. The exposed periosteum will re-epithelize. Problem is that muscle attachments may migrate downwards
True or false… yo usually must suture frenectomies like the lingual frenum
False
What is the best age to do a lingual frenectomy?
3-4
When do you do a vestibuloplasty?
Do this procedure when there are high muscle attachments but adequate bone.
How do you do a skin graft vestibuloplasty?
Obtain a split thickness skin graft from thigh
Put the skin graft under the denture, then wire the denture to the mandible. This will regenerate the attached gingiva. Sometimes may grow hair though…
What are osteotomies?
Taking a chunk of bone and moving it somewhere else
