What is Preeclampsia?
A
- Is a condition that causes high blood pressure during pregnancy and after labour.

What are the 4 categories of high blood pressure during pregnancy?
A
- Chronic hypertension
- Preeclampsia superimposed chronic hypertension
- Gestational hypertension
- Preeclampsia-eclampsia

What are Preeclampsia criteria?
A
- SBP : > 140 and DBP : < 90
- Proteinuria

Work up?
A
- CBC
- LFT
- BUN
- BMP
- Uric acid
- PT/PTT
- Fibrinogen

Symptoms of preeclampsia?
A
- Headache that is frontal
- Throbbing
- Visual disturbances ( such as scotomata )
- Epigastric pain
- Edema may be a sign as well

Differential diagnosis?
A
- Diabetes mellitus and pregnancy
- Antithrombin deficiency
- Cushing syndrome
- Graves disease
- Eclampsia

Treatment?
A
- In general, the mother and the fetus should be closely monitored.

Mild Preeclampsia treatment?
A
- Preterm and stable —> PO Labetalol and deliver at 37W.
- Term and unstable —> Induction of labor, IV magnesium sulfate and attempt vaginal delivery.

Sever Preeclampsia treatment?
A
- 24-34W and stable: PO Labetalol, deliver at 34W
- > 34W and unstable : Indiction of labor, IV magnesium sulfate and IV Labetalol

If delivered before 34 weeks what drug we give and why?
A
- We give Betamethasone one day to a week before delivery , to mature the fetus lungs, and prevent Respiratory distress.

- Renal failure
- Pulmonary edema
