Define PPH
Loss of >500ml blood within 24 hrs of birth
What are the 4 ts to consider as causes of PPH
Tone - rub fundus 70%
Trauma - attend to trauma 20%
Tissue - hospital to remove retained tissue 10%
Thrombin - hospital to replace thrombin <1%
Flow chart for PPH
If not adequate cerebral perfusion or radial pulse
Basic care - estimate blood loss - if none = regular placental care and management
If yes
- > 500ml = medstar activation / transport requirements / EOC for advice
- fundus (tone)
- direct pressure (trauma)
- empty bladder
- fluid - 250 aliquots to adequate cerebral perfusion and radial pulse
- consider external abdominal aortic compression
- Urgent transport
FLUID in PPH
250 ml aliquots toad adequate cerebral perfusion and radial pulse
Adequate = normal - patient should be alert and talking
Important points to consider with PPH
They are more hypovolaemic than they appear. NO WALKING or standing
Encourage breast feeding - release oxytocin and induce some contraction
IV access x 2
Continuous reassessment
