Post-Resus Care Flashcards
What is the main goal of resuscitation?
- Return pt. to normal cerebral function
- Normal haemodynamic Fx
- Est. & maintain stable cardiac rhythm
What is post-cardiac arrest syndrome?
- Brain injury
- Myocardial dysFx
- Systemic ischemia/ reperfusion response
- Persistent precipitating pathology
What approach to follow to Rx pt. immediately post-resus?
- ABCDE approach
Post-resus ABCDE approach - A & B - Aim:
- Clear airway
- Adequate oxygenation
- Adequate ventilation
Adult Post-Resus Care Guidelines - A & B:
- Sats 94-98%
- Insert advanced airway
- Waveform capnography
- Ventilate lungs to normocapnia
Which post-resus pt’s need intubation, sedation & controlled ventilation?
- Those w/ obtunded cerebral Fx
Post-resus ABCDE approach - C - Aim:
- Maintain normal sinus rhythm
- Maintain normal cardiac output
To adequately perfuse vital organs
Adult Post-Resus Care Guidelines - C:
- 12 lead ECG
- IV access
- Restore normovolaemia - crystalloid fluids
- SBP > 100 mmHg
- Inotropes/vasopressors to maintain SBP
- IABP monitoring
Post-resus ABCDE approach - D & E - Aim:
- Evaluate neurological Fx
- Ensure arrest not been ass. w/ other medical or surgical conditions needing immediate Rx
Adult Post-Resus Care Guidelines - E - Control Temperature:
- Constant temp: 32 - 36°C
- Sedation
- Control shivering
Which post-resus pt’s need targeted temp Mx?
- Those who remain comatosed after initial resus
What to do after post-resus ABCDE approach?
- Hx
- Monitoring
- Investigations
- Pt. transfer
Post-resus approach - Hx - Aim:
- Determine pt’s state of health
- Chronic medication
Post-resus approach - Monitoring - Aim:
- Continuous asssessment of vital organ Fx
- ID trends
Post-resus approach - Investigations - Which ones to run?
- FBC - anaemia
- Biochemistry - renal Fx, electrolytes, glucose, troponin, baseline
- ABG - Ventilation & oxygentation, acid/base balance
- 12 lead ECG - ACS, old MI, rhythm, baseline
- Echo - LV & RV Fx
