Post-op Flashcards
When can you start PROM for a RCR?
week 2
What are the PROM goals for week 2-4 of a RCR
90 flexion
90 abduction
30 IR/ER/Ext
What kind of exercises do you do for RCR in week 2-4
Scapular isometrics
What are the PROM goals for week 4-5 in RCR
120 flexion/abducition
30ER/Ext
45 IR
At what week can you initiate ER/IR PROM at 90 abduction
week 4-5
At what week can you initiate rhythmic stabilization exercise for RCR
week 4-5
At what week can a patient discontinue sling and build towards AROM w/ RCR
week 6
At what week can a RCR begin AAROM
week 5-6
At what week is there no PROM limitation for RCR
week 5-6
What is the criteria to advance from Phase 1 (week 1-week 6) to phase 2 in a RCR
Improving PROM
Flexion PROM to 125+
IR and ER PROM in scapular plane to 75+ (if uninvolved side is 80+)
Abduction PROM to above 90 in scapular plane
Phase two of RCR is what weeks?
What is the focus?
week 6-14
Dynamic stability
What is the criteria for advancing from stage 2 of a RCR (Week 6-14) to phase 3 (week 15-24)
Full AROM and PROM
Pain free w/ all strengthening exercises
dynamic shoulder stability
When can you begin to work on power for RCR
Phase 3 (week 15-24)
When can a RCR begin the return to sport phase?
week 25+
What is the criteria to advance from phase 3 (strength building phase week 15-24) to phase 4 (week 25+)
Maintenance of pain free ROM
Functional use of UE
Full muscular strength and power
What is the difference of the TSA and the RSA
TSA- used for a rotator cuff that can be repaired
RTSA- Ball and socket joint of the shoulder is now reversed. they have to rely on their deltoid now because they have poor rotator cuff integrity
How long should a TSA be in a sling?
3-4 weeks
What is the initial treatment for TSA (first 3-4 weeks)
PROM
AROM of elbow, wrist, hand,
Initial precautions of TSA
No extension or anterior capsule stretch (put pillow under elbow in supine)
No behind the back motion and no stretching into ER
What is the criteria to advance to phase 2 of TSA (week 4-6)
tolerates PROM
Achieves 90 degrees of flexion and abduction PROM (no combining)
Achieves at least 45 of ER
Achieves 70 IR
Criteria for TSA to progress to phase 3 (6 weeks)
Tolerates PROM/AAROM, and isometrics
PROM:
140 flexion
120 abduction
60 ER
70 IR
Able to elevate shoulder 100 degrees with good mechanics
When can a TSA start isometrics?
4-6 weeks
When can a TSA start moderate strengthening?
6+ weeks
At 6 weeks what is the lifting limit of a TSA
No lifting objects heavier than 6lbs
When can a TSA initiate behind the back AAROM
6weeks (phase 3)
How long is a SLAP repair in a sling
4 weeks
When can a SLAP repair work on biceps strengthening
Phase 3, 12-20 weeks
When can a cervical fusion return to driving
when they stop taking narcotics
When can a cervical fusion return to running/jogging/riding horses
6 months
What do we want to avoid for cervical fusion patients
Avoid lifting over 20 lbs for 2 months
Cervical AROM or PRONE exercises
Avoid running/jogging/horse riding for 6 months
