Legal requirements for a vet Rx

A
  1. Veterinary surgeon details, name, address, tel no/ qualification, signature (If CD sch 2 or 3 then RCVS number needs to be present)
  2. Animal: name and species (including address if it is different from the owner)
  3. Owner: Name and address
  4. Medicine details: Name, strength, form, dosage
  5. Administration instructions, and warnings e.g. withdrawal period if necessary
  6. If repeatable, number of repeats
  7. If human medicine/unlicensed then must have “For administration under the veterinary cascade
  8. For CD2 and CD3 must state “This item has been prescribed for an animal or herd under the care of a vet”
  9. Date, 6 month expiry, including repeatable Rx, all supplies must be made within 6 months. If CD 2,3,4= 28 days

Vet medicine dispensing label

A

-If a medicine is not prescribed under the cascade it is not legally required to have a dispensing label
Must contain the following details, unless they already appear on the product packaging:
1. Name of prescribing veterinary surgeon
2.Name and address of the owner
3. Name and species of animal
4. Medicine details: Name, strength dosage, and administration instructions
5. Necessary warnings or withdrawal period+ any special storage instruction
6. For animal treatment only
7.Pharmacy name and address
8. Date of supply, and expiry date of medicine
9. Keep out of reach and sight of children

A

1) Vet medicine licensed for another species or condition
2) UK licensed human medicine or EU licensed veterinary medicine
3) Specially manufactured or extemporaenous medicine (unlicensed)
