Which CD’s can a midwife obtain using the midwife supply order?
- Pethidine
- Diamorphine
- Morphine
What must a midwife supply order contain?
- Name of midwife
- Occupation of midwife
- Name of the person to who the CD is being administered to or supplied
- Purpose for which the CD is required
- Total quantity of the drug to be obtained
- Signature of an appropriate medical officer
Can a Dr or dentist be supplied with a Sch 2 or 3 CD in an emergency?
Yes
On the premise that a requisition will be supplied in 24 hours
Failure to do so would be an offence on the part of the Dr or dentist
Written authorization and stock collection for CDs
- If stock is collected by a messenger on behalf of a purchaser a written authorisation must be provided to the supplying pharmacist that enables the messenger to recieve the supply on behalf of the purchaser
- pharmacist must be satisfied that it is legit
- must be retained for 2 years
Sch 1 CD
- Most have no therapeutic use
- Generally, a license is required for their production, possession or supply
E.g LSD
Sch 2 CD
- Pharmacists and other classes of person named in the 2001 regulations have a general authority to possess, supply and procure
- Opiates e.g. diamorphine, morphine, methadone, oxycodone, pethidine, major stimulants, e.g. amfetamine, quinalbarbitone and ketamine
- 28 day Rx expiry
- Address of prescriber required to be in the UK
- Prescripton requirements needed
- License required to import or export
Sch 3 CD
- CD no register POM
- Include minor stimulants and other drugs (e.g. buprenoprhine, temazepam, tramadol, midazolam, phenobarb)
- Gabapentin and pregabalin= rescheduled to sch 3
- 28 day Rx expiry
- Address of prescriber required to be in the UK
- Invoices need to be retained for 2 years
- Rx requirements needed
- License required to import or export
Sch 4 CDs PART 1
- CD benz POM
- Most BZDs e.g. diazepam
- Non BZDs hypnotics/Z drugs (such as zopiclone)
- 28 day Rx expiry
- Address of prescriber NOT required to be in the UK
- Invoices need to be retained for 2 years
- License required to import or export
Sch 4 CDs PART 2
- CD Anab POM
- Contains most of the anabolic and androgenic steroids
- Clenbuterol (Adrenoceptor stimulant)
- Growth hormones
- 28 day Rx expiry
- Address of prescriber NOT required to be in the UK
- License required to import or export (Unless the substance is imported or exported by a person for self administration)
Sch 5 CDs
- Codeine
- Pholcodeine
- Morphine
- 6 month Rx expiry date
- Address of prescriber NOT required to be in the UK
- Invoices need to be retained for 2 years
Travelling with CDs
- If the person is travelling with less than 3 months supply then a personal license is not required by the Home Office
- It is advised that a covering letter signed by the prescriber is obtained, that confirms the name of the patient, travel plans, name of the prescribed CDs, total quantities and dose

3) Address of recipient
4) Profession or occupation
5) Total quantity of drug
6) Purpose of the requisition
Possession of Sch1 CDs
- Home license required
- However some pharmacists (particuarly those working in hospital) can possess without HOL under the following exemptions:
1) Possession is taken for destruction
2) The purpose of handing over to the police
Patient’s confidentiality should normally be maintained and police should be called on the agreement that the source will not be identified. If there is suspicion that the person could be dealing then pharmacist may decide that the greater interests of the public require identification of the source
Processing requisition forms (marking and sending)
When recieved what are the legal requirements?
- Mark the requsition indelibly with the suppliers name and address (i.e name of the pharmacy) if a stamp is used this must be clear and legible)
- Send original requisiton to the relevant NHS agency
Good practice:
- pharmacies should retain a copy of the requistion for 2 years from date of supply
Technical errors on CD scripts
-Cannot correct other amendments or omissions (e.g missing date, incorrect dose form or strength) These should be corrected by the original prescriber or, in an emergency, another prescriber authorised to prescribe CD’s
