Polymyalgia Rheumatica Flashcards
Define polymyalgia rheumatica (PMR)
Inflammatory rheumatological syndrome that manifests as pain and morning stiffness involving the neck, shoulder girdle, and/or pelvic girdle in individuals older than age 50 years.
Epidemiology
Increases with increasing age
Esp in females
Esp in northern European populations
Aetiology/risk factors
Associated with giant cell arteritis
Female
Over 50
Which sites are affected?
Neck
Shoulders
Pelvis
Symptoms
Pain in neck, shoulder or pelvis
Asthenia
Malaise
Depression
Constitutional symptoms: Low grade fever Anorexia Weight loss Oligoarticular arthritis
What is asthenia?
Physical weakness
How quickly does the pain develop and when do patients present?
Acutely, however patients don’t present until a few weeks later
What is oligoarticular arthritis and which joints are typically involved?
Less than 4?
Wrists - MCP
Knees
Investigations
ESR > 40mm/hr (however this might be normal in 20%)
CRP elevated
FBC (other myeloproliferative diseases can masquerade)
Ultrasound
Why do you do an ultrasound?
Bursitis is commonly found in PMR, especially trochanteric bursitis
Treatment (1st and second line)
1st line: 15mg oral prednisolone
2nd line: intramuscular methylprednis
+ calcium, vit D, bisphosphonates e.gl alendronic acid
When do patients respond?
Usually gets better within 20 hrs however treatment for at least a year is recommended to prevent relapse
Give four complications of steroid therapy
T2DM
Osteoporosis
Cataracts
Avascular necrosis
What do you need to screen for?
Giant cell arteritis
Diagnosis
High ESR
Typical symptoms
