What is it?
abnormal collection of air in the pleural space that causes uncoupling of the lung from the pleural space (DEFLATION OF THE LUNG)
What is primary spontaneous pneumothorax (PSP)?
a pneumothorax that occurs without any precipitating event and in the absence of clinical lung disease
What ages are commonly affected by PSP?
in 20s
What is secondary spontaneous pneumothorax (SSP)?
a pneumothorax that occurs under the complication of an underlying disease
What ages tend to be affected by SSP?
60-65 yr olds
What is the risk factor for spontaneous pneumothorax and what causes it? (4,1)
- tall
- thin
- male
- 20s
CAUSE
- rupture of sub-pleural bulla
What underlying medical condition tends to cause a pneumothorax?
COPD
What are the symptoms? (2)
- sudden onset of unilateral pleuritic pain
2. progressively increased breathlessness
What are the signs? (3)
- reduced expansion
- hyperressonance on percussion
- diminished breath sounds on affected side
What sign will you see in tensional pneumothorax?
- tracheal deviation away from the pneumothorax
Differential diagnosis
- pleural effusion
- chest pain
- PE
Investigations (4)
- CXR
- CT
- US
- ABG
When would you do an ABG?
if hypoxic
Treatments (4)
- chest drain
- oxygen
- thoracocentesis/ pleural tap
- surgery - PLEURODESIS, pleurectomy, pleural abrasion, bullectomy
