A
- Spontaneous (especially in young thin men) due to rupture of a subpleural bulla
- Chronic lung disease - asthma; COPD; cystic fibrosis; lung fibrosis; sarcoidosis
- Infection - TB; pneumonia; lung abscess
- Traumatic
- Carcinoma
- CTD - Marfan’s syndrome, Ehlers–Danlos syndrome

Acute Mx of primary pneumothorax:
A
- SOB and/or rim of air >2cm on CXR - YES
- Aspiration - successful? - NO
- Consider repeat aspiration - Successful? - NO
- Chest drain

Acute Mx of secondary pneumothorax:
A
- SOB and age >50 and rim of air >2cm on CXR? - YES
- Chest drain

Signs tension pneumothorax:
A
- RDS
- Tachycardia
- Hypotension
- Distended neck veins
- Trachea deviated away from side of pneumothorax
- Increased percussion note
- Reduced air entry/breath sounds on the affected side
