- COPD
- Heart failure
- Anaphylaxis

A
- Foreign body or tumour
- Acute epiglottitis (younger patients)
- Anaphylaxis
- Trauma, eg laryngeal fracture

Crepitations - Diff Dx:
A
- Heart failure
- Pneumonia
- Bronchiectasis
- Fibrosis

Chest clear but breathless - Diff Dx:
A
- Pulmonary embolism
- Hyperventilation
- Metabolic acidosis - DKA
- Anaemia
- Drugs - salicylates
- Shock - may cause ‘air hunger’
- Pneumocystis jirovecii pneumonia
- CNS causes

Key investigations in breathless pt:
A
- Baseline observations - O2 sats, pulse, temperature, peak flow
- ABG if saturations <94% or concern about acidosis/drugs/sepsis
- ECG (signs of PE, LVH, MI?)
- CXR
- Baseline bloods: glucose, FBC, U&E, consider drug screen
