fluid moving across pleural space - inc venous congestion or hyponatraemia

R sided pleural effusion w ovarian malignancy
presentation
SOB pleuritic chest pain dullness to percussion reduced breath sounds reduced expansion tracheal deviation
CXR findings
blunting of costophrenic angle
fluid in lung fissures
meniscus - curving upwards to meet chest wall and mediastinum
tracheal + mediastinal deviation
Mx
conservative if small - treat underlyign cause
pleural aspiration
chest drain
empyema
infected pleural effusion
when to suspect empyema
pt w improving pneumonia but new/ongoing fever
empyema - pleural aspiration findings
pus
acidic
low glucose
high LDH
empyema Mx
chest drain
antibiotics
