-process utilized after collection of data to establish priorities and set objectives for the community/population served
-can occur on local, state, national, or global level
community assessment purpose
-identify the CH problems that are priority for intervention as well as identify resource within the community to address health problems
-includes i.d. of assets & strengths & weaknesses to address, establish priorities to address, and identify health needs for communities
coalitions
-organization of diverse interest groups that join their human and material resources to produce a specific change they they are unable to deliver as independent individuals or separate organizations
-need clear mission, goals, objectives, and expectations
-needs good mgmt of meeting, respectful of time, and invitation of people who have expertise in project
what to do when forming a coalition
-make use of expertise of individual members by asking for help and input on matters r/t their field
-seek out people with a range of opinions and roles in community & find our who has been a valuable member of previous community coalitions or teams
-consider how each member of coalition will benefit from the work of the coalition
-understand group dynamics and remember that people agree to serve on coalitions more than just to feel good about helping community
-try to have a heterogenous group but one where members are respectful of other’s ideas
how to efficiently run a meeting
-know purpose of meeting
-show respect of time and expertise of members
-send agenda and background materials in advance
-get right people to attend
-invite only key stakeholders
-stay on topic
-communicate results
-learn how to resolve conflicts and reach consensus
change theory/Lewin’s Model of Change
-unfreezing: moves a community from stage of denial or lack of awareness of need to change to a state of preplanning or prep for change
-changing: when understanding of what needs to be changed and how the change will take place
-refreezing: process of stabilizing once a change has occurred with the goal of sustaining the change
community readiness for change
-perform assessment and their readiness to change r/t specific issue
-success for change depends on well conceived interventions and implementation plans
-important to select appropriate interventions and consider resources available including people, money, facilities, and time
SMART objectives
-specific: what behaviors, knowledge, skill, change in health status indicators will result?
-measurable: how will it be measured and how will one know if it’s achieved?
-achievable: is it realistic with resources and time available to the program?
-relevant: r/t programs goal and achievement?
-time bound: when will it be achieved?

-focus on what happened, how well, why, and what the results were
-determine indicators or measures that will be used to answer questions
-what method to collect data
-specify time frame to collect data
-how will you analyze
-how will communicate results

-goal is to change health behavior
