What does each grade of the Wagner Grade Classification System mean?

A

0: No open lesion, but may posses pre-ulcerative lesions; healed ulcers; presence of bony deformities

1: Superficial ulcer not involving subcutaneous tissue

2: Deep ulcer with penetration through the subcutaneous tissue; potentially exposing bone, tendon, ligament or joint capsule

3: Deep ulcer with osteitis, abscess or osteomyelitis

4: Gangrene of digit

5: Gangrene of foot requiring disarticulation

What does each grade of the Modified Ashworth Scale mean?

A

0: No increase in muscle tone

1: Slight increase in muscle tone marked by a catch and release or by minimal resistance at the end of the ROM when the affected part is moved into flexion or extension

1+: Slight increase in muscle tone marked by a catch and minimal resistance through less than half of the remaining ROM

2: More increase in muscle tone marked by minimal resistance through more than half of the ROM, but affected part(s) moved easily

3: Considerable increase in muscle tone, passive movement is difficult

4: Affected part(s) rigid in flexion or extension
